Did you receive your $50 e-gift card yet?
This is a reminder that the physician qualification form and health assessment are due on May 31, 2019, for those employees enrolled in Blue Cross with NMC.
If you have already completed your requirements, please make sure you check your Rewards page on your BCHW portal to verify you received credit for your activities. In order to receive credit and earn your $50 e-gift card for your completed activities, you must have a registered account on bcbsm.com.
