Just another reminder that you need to complete the blue Coordination of Benefits form from your open enrollment packet if you enrolled in the medical or dental plans for 2014. This form can be sent directly to Meritain Health at the address on top of the form, or sent interoffice to HR and we will send it on.
If you do not complete this, you will be contacted by Priority Health and/or ASR when you actually have claims in 2014 because they must verify if there is any other coverage for these benefits. Sending in this form should help avoid this additional step.
