The following is information regarding your 2015 benefits.
Blue Cross Blue Shield of Michigan ID Cards
They will be arriving in the mail by the end of December. If you enrolled in medical and dental benefits for the same number of family members in each plan (example full family for medical and full family for dental) you will only receive one card, which covers both. You will receive two separate cards if you enrolled a different number of family members in each plan. For example, your full family in medical benefits, but only yourself in dental (or any combination), BCBSM will be sending you two separate cards in the mail.
Eye Med ID Cards
They will be mailed to your home by the end of December.
Flexible Spending Account changing to Meritain Health for 2015
Reminder to access the information book regarding Flexible Spending Accounts and fill out the Direct Deposit Authorization Form on page 13 if you are interested in taking advantage of this feature.
If you have been using a Flexible Spending Account in 2014, Priority Health will continue to manage those funds.
Health Savings Account (HSA) Information for 2015
If you already have an HSA set up and have been receiving deposits from NMC you do not need to do anything further- we will use this information.
If you are new to the High Deductible Health Plan in 2015, you need to set up a Health Savings Account at the bank of your choice. Please provide documentation from the bank including the routing and account numbers. This should be sent to Kelly Westphal in HR, by December 15.
**IMPORTANT NOTE- If you have a current Flexible Spending Account that still has money in it, you have to expend that account before you can open a Health Savings Account.
Life Insurance Medical Form (Evidence of Insurability)
If you requested additional voluntary life insurance that requires you to fill out medical paperwork, please turn that into Hollie in HR by December 19, so it can be sent to Cigna for processing.
