Once you\u2019ve completed your assessment you\u2019re one step away from a $50 gift card. Just log in to <strong>bcbsm.com<\/strong> or the Blue Cross\u00ae Blue Shield\u00ae of Michigan mobile app and click <em>Health &amp; Wellness<\/em> to get started.<\/p>\n<p>NMC teammates that elected our Blue Cross Blue Shield of Michigan insurance and complete the online Health Assessment and submit the Physician Health Screening form before <strong><u>May 31, 2018<\/u><\/strong> will receive a $50 gift card.<\/p>\n<p>Don\u2019t wait too long!
